DISTRICT PHYSICALS, ONE NIGHT ONLY

It is required that all students complete a physical prior to participation in their first 7th, 9th, and 11th grade practice in interscholastic activities. Physicals are also required for students participating in weights, aerobics, dance, drill, cheer, or/and bowling. All students in Minidoka County School District are required to have the physical on file. Any questions or concerns can be directed to Activities Director- Tim Perrigot 436-4721 ext 202.

DATE: GIRLS - Weds. May 27, 2009 and BOYS - Weds. May 27, 2009
TIME: GIRLS - 5:00 P.M. - 6:00 P.M. and BOYS - 6:00 P.M. - 7:00 PM
WHERE: Mini-Cassia Occupational Health, 1218 9th St. Suite # 13
Pre-Screening forms available at your school only. Forms will NOT be available at Occupational Health
FEE: $10.00 - or appropriate signature on Community Service Coupon !!
*** MUST bring Pre-screening form Completely Filled Out WITH parent signature and name of family physician.....
*** This is a medical screening for school sports only. It does not take the place of a regular physical by your family physician.
We Will Not Allow Boy Scout Physical Forms
